The Promoting Life Together (PLT) Collaborative was a 20-month life promotion and community wellness initiative convened by the Canadian Foundation for Healthcare Improvement, now Healthcare Excellence Canada, and in response to and with members of the Canadian Northern and Remote Health network
The PLT Collaborative modeled a co-designed learning approach that brought together six multi-disciplinary teams from across northern, rural, and remote parts of Canada. Each team in the Collaborative worked alongside communities and First Nations and Métis coaches to deliver an initiative that addressed life promotion and community wellness for people living in northern and remote regions from coast to coast to coast.
